Score is the adjusted gross score (each hole capped at net double bogey); rating and slope are printed on the scorecard or tee sign.

Slope rating (55 – 155) of the course you plan to play, used with the rating below to convert your index into a course handicap. Leave both blank to skip that step.

Course rating of the target tees — the score a scratch golfer is expected to shoot. Used together with the slope to compute your course and playing handicap.

Par of the target course. It enters the course handicap formula: index × (slope ÷ 113) + (rating − par).
